Prefix of single letter X, plus a serial number on top right and bottom left. Date of issue in black on bottom left. Head office ‘Port of Spain Trinidad’ in black on bottom right. Capital letter T in field on top right. Manuscript signature, on bottom left of ACCOUNTANT and printed signature on bottom right of MANAGER.
